What makes Agape one of the best rated assisted living homes near Concord?

Agape is a small, family-run care home for elderly residents — not a large corporate facility. Our 5-star Yelp rating reflects the real experiences of families who trusted us with their loved ones.

How many residents do you care for?

We are a boutique residential care home licensed for only 6 residents. This small ratio ensures that our staff can provide immediate, highly personalized attention that larger facilities often cannot match.

Care & Daily Support

Do you provide dementia care and Alzheimer's care in Concord?

Yes. Agape Assisted Living specializes in dementia care and Alzheimer's care for elderly residents in Concord, CA and throughout Contra Costa County. Our staff are trained in memory care techniques and provide a secure, structured daily routine that promotes comfort, dignity, and cognitive engagement for residents at every stage of dementia or Alzheimer's disease.

Do you offer post-rehab senior care and recovery support?

Yes. We provide post-hospital and post-surgery recovery support for elderly residents after hip replacements, strokes, falls, and other medical events. Our assisted living home in Concord, CA helps seniors regain strength, mobility, and independence in a comfortable, homelike setting — a better alternative to a large clinical rehab facility.

Can residents age in place at Agape without having to move?

Absolutely. One of the biggest benefits of our assisted living home is that residents never need to relocate as their care needs change. We provide five distinct levels of care — from basic personal assistance all the way through advanced dementia care, memory care, and hospice coordination. Your loved one stays in the same familiar home, with the same caregivers, for as long as they need.

What levels of care do you provide at Agape?

We offer five distinct Levels of Care, ranging from Level 1 (basic personal assistance) to Level 5 (full care including hospice coordination). This includes assisted living, dementia care, Alzheimer's care, memory care, and post-rehab senior care — all under one roof so your loved one can age in place comfortably.

Is staff available 24 hours a day for medication and mobility?

Yes. We provide continuous 24-hour senior care and supervision. This includes assistance with activities of daily living (bathing, dressing, mobility) and careful medication management for all elderly residents.

Admissions, Cost & Tours

Is Agape Assisted Living currently accepting new residents?

Yes, we are currently welcoming new residents. Space is limited as we maintain a small, homelike environment. We encourage families to reach out as early as possible to discuss availability and begin the admission process.

How do I start the admission process?

The first step is to contact us by phone at (925) 788-2530, via WhatsApp, or through our contact form. We will schedule a tour, discuss your loved one's care needs, and walk you through the required California state admission forms and our Admission Agreement.

How much does assisted living at Agape cost?

Pricing varies based on the level of care required for each resident. We encourage you to contact us directly at (925) 788-2530 or via our contact form for a personalized quote. We are happy to discuss options that fit your family's needs.

Do you offer tours for families in the Concord area?

Visiting & Family Communication

Can family members visit residents at Agape?

Absolutely. Family involvement is something we deeply encourage. Visiting hours are Monday through Friday 10:00 AM – 6:00 PM, and Saturday and Sunday 10:00 AM – 5:00 PM. We welcome families to be a part of their loved one's daily life.

Understanding the Terminology

Is Agape a board and care home?

Yes. Agape Assisted Living is a licensed board and care home — formally a Residential Care Facility for the Elderly (RCFE #75601346) — in Concord, CA. Board and care homes are small, family-run residential settings that provide personal care, meals, and supervision. Unlike large assisted living complexes, our board and care home serves only 6 residents, ensuring highly personalized attention in a true home environment. We are not a nursing home or skilled nursing facility.

What is the difference between a board and care home and a nursing home?

A board and care home like Agape provides personal care, meals, and supervision in a warm residential setting — it is not a nursing home or skilled nursing facility (SNF). Nursing homes deliver licensed nursing and medical care around the clock for residents with complex clinical needs. Agape is ideal for seniors who need help with daily living, dementia or memory care, or post-rehab recovery in a homelike environment rather than a hospital-style facility.

Practical Questions Families Ask on a Tour

Do you accept long-term care insurance or Medi-Cal?

Most long-term care insurance policies cover care in a licensed RCFE, which Agape is — check your policy's daily benefit amount and elimination period. California's Medi-Cal Assisted Living Waiver can help eligible low-income seniors pay for residential care in participating facilities, and wartime veterans or surviving spouses may qualify for VA Aid & Attendance. Call us to talk through what applies to your situation rather than assuming something is not covered.

Can residents bring their own furniture and decorate their room?

Yes, and we encourage it. Familiar furniture, bedding, photographs, and keepsakes make a room feel like home rather than a placement, and for residents with memory loss familiar objects genuinely help with orientation and comfort. We will talk through what fits the room and what needs to stay clear for safe mobility.

What happens in a medical emergency?

Caregivers are awake and on site 24 hours a day, so a resident is never waiting to be found. We call 911 for anything urgent, notify the family immediately, and contact the resident's physician. Emergency contacts, physician information, and treatment preferences are documented for every resident at admission and kept current.

Can a married couple live at Agape together?

It depends on room availability and each person's care needs at the time — with six beds total it is not something we can promise in advance. Please call and ask. If we can make it work we will, and if we cannot we will tell you honestly rather than separate a couple by surprise later.

Do you accommodate special diets?

Yes. All meals are cooked on site, so diabetic, low-sodium, soft, and texture-modified diets are handled in the normal course of cooking rather than as an exception. We follow physician-ordered dietary restrictions and also work around plain preferences — a resident who has never liked fish is not going to start now.

What happens if my loved one needs hospice care?

Residents can stay at Agape and receive hospice care here. We coordinate with the hospice provider of your choosing, who visits and manages comfort care while we continue the daily caregiving your loved one already knows. Not having to move somewhere unfamiliar at the end of life is one of the most meaningful things a small home can offer.

How will I hear about changes in my parent's condition?

We call you. Families are told about falls, illness, medication changes, behavioural changes, and physician visits — not left to discover them at the next visit. You can reach us by phone or WhatsApp any time, and because the same small group of caregivers is with your parent every day, the person you speak to actually knows them.

Can family bring a pet to visit?

Usually yes for a well-behaved pet, and it is often the highlight of a resident's week. Please arrange it with us beforehand so we can check it works for the other residents — allergies and a fear of dogs are both real considerations in a shared home.
