Choosing a Residential Care Facility for the Elderly (RCFE) is one of the most important decisions a family makes. Here's a practical guide to what to look for, what to ask, and what red flags to watch for.
What Is an RCFE?
A Residential Care Facility for the Elderly (RCFE) is a state-licensed facility that provides 24-hour non-medical care and supervision for seniors aged 60 and older. In California, RCFEs are regulated by the California Department of Social Services (CDSS) Community Care Licensing Division.
Unlike skilled nursing facilities (SNFs), RCFEs focus on personal care assistance, supervision, and a homelike living environment. They can range from large institutional campuses to small residential homes with just 6 residents. In Contra Costa County, you'll find both — and the difference in day-to-day care experience can be significant.
Questions to Ask During an RCFE Tour
Never sign an admission agreement without visiting in person. During your tour, ask:
- Is the facility currently licensed by the California CDSS? Can I see the license?
- What is the current staff-to-resident ratio during the day? At night?
- How long have the primary caregivers been employed here?
- How is medication managed and documented?
- What is the process if a resident's health declines? At what point would a resident need to leave?
- What activities are offered, and how are they adapted for residents with dementia or limited mobility?
- Are meals prepared on-site? Can dietary restrictions be accommodated?
- What is the policy on family visits and communication?
- How are incidents — falls, medical events — documented and communicated to families?
- What happens if I need to give notice and move my loved one?
What to Look for in Staff
The quality of an RCFE is determined almost entirely by its caregivers. During your visit, observe:
- Do staff members greet residents by name and engage with them genuinely?
- Do residents appear calm, clean, and comfortable?
- Is the atmosphere warm and relaxed — or tense and rushed?
- How does staff respond when a resident asks for help?
- Do the owners or managers work on-site regularly, or is the facility absentee-run?
Red Flags to Watch For
Walk away from any facility where you observe:
- An inability or reluctance to show you the CDSS license
- Residents who appear unkempt, unwashed, or visibly distressed
- Staff who do not greet or engage with residents during your visit
- A strong odor of urine or poor sanitation in common areas
- Pressure to sign an admission agreement on the same day as the tour
- Vague or inconsistent answers about staffing ratios
- Any active citation from CDSS for health or safety violations
